1. In small groups, propose a strategy for how government and non-government organisations could work together to achieve person-centred healthcare for one the following health conditions; cardiovascular disease, cancer, mental health issues or dementia. The strategy should consider:

 

  • Accessibility of healthcare (e.g. reducing barriers for underrepresented populations).
  • Continuity of care (e.g. integrating services from hospital to home care).
  • Patient and caregiver involvement (e.g. support groups, patient education).
  • Improving quality of life (e.g. holistic care that includes mental, emotional, and social support).
  • Identification of key players (e.g. hospitals, doctors, social workers, local community organisations, health insurers).
  • Identification of possible challenges or barriers in collaboration and propose solutions to overcome them.
